The Care Team

Location: Stillorgan, Dublin

The Care Team provides primary care services to clients in their own home. The organisation is owned and managed by nurses in partnership with GPs around the primary care sector. 
The Care Team is the first multi-professional organisation in Ireland to provide comprehensive homecare. The Care Team promote and follow a client-centred approach to care at home. We encourage an open and questioning environment that promotes a safe and high quality service. Our team have an opportunity to positively influence the course of our client’s illness and recovery. 
All our home care and home healthcare packages are developed in collaboration with the multidisciplinary agency. Specialist packages available for care at home and are based on best practice recommendations.
We work with both the private sector and with the HSE.

Disability Support Worker (Dublin)
Description: Contact Care provides quality person centered care to service users in their own home and community setting. Currently in its tenth year the organisation is in a massive expansion phase. Contact care provides a wide range of services such as: Disability services to children/adolescent/adults with physical and/or intellectual disabilities Contact Care adapt a social care model to care delivery as defined by the Philosophy of Independent Living. Our clients are located throughout Dublin. Contact Care is an exciting dynamic nurse led organisation that work as a team and we believe in supporting and listening to our employees. Role: This role involves working with a range of ages child, adolescent and adult in their home, learning environment and/or community setting on a particular structured activity programme. A plan for support will be set out in partnership with the family and a Client Care Coordinator and Multi-disciplinary team. These services are usually delivered after school hours and there may be a requirement for occasional Saturdays. This role involves working in partnership with the service user, their family and other supports as outlined within their individual care plan. The role involves the promotion of independence, choice, and wellbeing to enable the child/adolescent/adult enjoy the highest quality of life within their local community. We would like to hear from enthusiastic, motivated people who have a genuine desire/interest in working in this area.
