Client Care Coordinator
The Award-Winning Contact Care has an opportunity for a full-time Client Care Coordinator to join our busy team. The role is community based and requires a highly organised, flexible self-starter with excellent time management skills and good initiative. This is a hands-on management role based in the community. The individual will need to have an in-depth understanding of social care and case management. Role and Responsibilities include: Coordination of care packages - requiring communication with the HSE, PHNs, GPs and hospital multi-disciplinary teams. Assessing service users for homecare packages and developing recommendations to the relevant stakeholders. Case management identifying needs and requirements for services users, providing assistance in achieving goals for service users to remain in the community. Caring directly for our service users, carrying out care assessments and drawing up care plans. Managing a team of supervisors and healthcare assistants. Introducing them to service users, shadowing care shifts to ensure the quality of care and up skilling them in some clinical areas. Requirements: Relevant qualifications in Intellectual Disability Studies/Social care/Nursing Minimum of 2 years or more of Community Healthcare Experience Intellectual Disability Caring Experience Desirable Good IT skills in Microsoft office, outlook. Proven Supervisory skills Excellent communications skills Current CPR certification Current people moving and handling certification Working experience of Patient Assessments and Care planning Full Irish Driving Licence Salary on application. Full-Time Healthcare Assistant

We look after a wide range of adults in the community with long term disabilities requiring complex care to personal and social care of the older person. We are looking for an experienced, dynamic caring person to work with our clients. This role is a full time 37.5 hours p/w salaried position with company pool car provided. Why should you apply? The role is community based in Dublin and requires a highly organized, flexible self-starter with excellent time management skills and good initiative. This is a hands-on management role based in the community from Monday to Friday 9 to 5 job hours and few late evenings weekend work. The successful applicant will be capable of managing all care aspects of day to day operations, be responsible and accountable, possess excellent communication skills and confident to lead the implementation all quality care initiatives.
