Is your Slim PS4 stuck offline, because it won't upgrade to latest firmware (giving error SU-42118-6) and the disc drive doesn't seem right?
This pack of two (2) fuses should fix you up as long as only one or both of fuses at F6201 and / or F6202 have blown open, there is no life from the disc drive when trying to insert a disc, and there is no other damage to the bluray drive / console*
Each fuse is 1mm x 0.5mm in size, so quite hard to handle / install / solder / remove

*If the Bluray Drive controller IC has gone, replacing the fuse won't fix it. Remember the way PS3 and old PS4 drives and daughter boards were married / paired up? Well the same is true of Slim / Pro Disc Drives and their corresponding IC chips on the machine motherboard. This kind of fault will be beyond repair for me, and most repair shops.
I have fixed 6 out of 10 slims that had this fault, one Pro had the blown fuse, but could not be fixed due to faulty IC chip
So, chances are 60% or better if we are dealing with a Slim where you know what might have caused the problem. E.g. drink spilled in to machine, quite likely just the fuse went.
However machine never been harmed, then it might well be the IC chip, not solvable by fuse replacement
