The Jailbreak gets reset every time the console gets powered off or restarted, to re-initiate the jailbreak you have to launch the Vue Free app each time, which will automatically re-apply the jailbreak. If this doesn't work, restart the console and try again.
Don't connect the PS4 to the internet unless you know how to keep the console from updating, follow guide below for information.

Even when connected to the internet safely, make sure to not update the Vue Free app or the jailbreak will be lost, AND DEFINETALLY to not update the system version as that is not reversible and will prevent future jailbreaking.
